What Is the Support at Home Program?
The Support at Home program is part of Australia’s aged care reform, designed to make care simpler, fairer, and more flexible for older Australians who wish to live independently. Through our partnership with Trilogy Care, Astute Living Care will help you understand your classification, manage your quarterly budget, and maximise your funding.
Instead of four home care package levels, the new system offers:

Three Service Categories:
Clinical Care – for medical and nursing needs.
Independence Support – for mobility, self-care, and daily living.
Everyday Living Support – for cleaning, cooking, and social activities.

New Client Types:
Grandfathered (current clients continuing their package)
Hybrid (transition clients)
New (clients entering under Support at Home)

From November 2025, all existing Home Care Packages will transition automatically to the Support at Home system. You won’t lose services or funding — your plan will simply become easier to manage.
Older Australians who need help with daily activities, health management, or mobility may qualify for government-funded Home Care Packages (HCP) through My Aged Care. Our team can guide you through the eligibility and application process step-by-step.
Depending on your classification (Grandfathered, Hybrid, or New), your contribution may be adjusted — but most clients will see no major increase. Our team will explain your exact costs.
We’ll manage everything for you — from funding adjustments to care planning. You’ll continue receiving consistent care, with the added benefit of transparent quarterly reporting.
